Its ACO partners collectively saved Medicare $337 million in 2024, making it the largest and top-performing hospital-led enabler in the MSSP. Notably, Wellvana’s flagship ACO generated more Medicare savings than any other MSSP ACO in the country that year.

Cook County Health Partnership
Cook County Health, the public health system serving the Chicago metropolitan region, has also partnered with Wellvana. This collaboration extends value-based care capabilities to one of the most diverse patient populations in the nation.
Supporting Rural Healthcare Through Cibolo Health
One of the most significant expansions involves Cibolo Health, an organization that supports independent rural hospitals through provider-owned, state-based clinically integrated networks (CINs). Cibolo currently operates rural CINs in six states.

Rural providers face unique challenges: limited resources, geographic isolation, and the need to preserve local independence while meeting national quality standards.
