The strategic partnership primarily targets Humana’s Medicare Advantage HMO and PPO members grappling with chronic kidney disease (CKD) or end-stage kidney disease.
According to Robert Sepucha, CEO of Interwell Health, kidney disease’s intricate nature and costly treatment necessitate innovative solutions. The joint effort, coming within the first year of their merger, leverages the extensive network of nephrologists to advance patient health outcomes and significantly cut down overall care expenses for payers.
This initiative spans Arizona, Arkansas, Colorado, Kansas, New Mexico, New York, North Carolina, Ohio, Oklahoma, Pennsylvania, South Carolina, Texas, and West Virginia. Humana’s Senior Vice President, Carl Daley, stressed the organization’s dedication to delivering comprehensive support and tailored care to its members. With millions afflicted by chronic kidney disease, this partnership expands programs that furnish value-based, coordinated care and resources tailored to every phase of the healthcare journey.
Humana and Interwell’s alliance echoes a broader trend in healthcare, where companies embrace value-based care models to enhance patient outcomes. This program encompasses over 1,700 network nephrologists, renal care coordinators, and virtual support specialists. In a recent expansion, Interwell Health added 20 new physician practices, further extending its network to cover 125,000 members.

The partnership demonstrates a path to improved outcomes while simultaneously reducing the total cost of care. Hart attributes their success to a dedicated, interdisciplinary care team working closely with a nationwide network of high-performing nephrologists.
