EHR vendor MEDITECH’s population health tool will leverage aggregated patient data through Expanse Population Insight to help healthcare organizations gain a complete view of their patient population health needs.
“As evolving payment models and key industry drivers like health equity and consumerism continue to reshape healthcare delivery, our customers need solutions that help them see the full patient picture, regardless of where a patient receives care,” Helen Waters, executive vice president and COO at MEDITECH, said in a press release.
“Expanse Population Insight enables organizations to accelerate their population health initiatives,” Waters continued. “Care providers can leverage a larger and more powerful data asset embedded within their native EHR workflows. This way, they can cater to the specific needs of their communities.”
Expanse Population Insight will use aggregated patient data from claims, EHR, and other sources to offer clinicians insights regarding patient population risk, care gaps, and care utilization, MEDITECH stated.
Additionally, integrating this information into EHR workflows will allow for the creation of cohorts on registries by disease state, provider attribution, risk contract, and numerous other criteria, the press release stated. Through this, the care manager can implement outreach strategies and interventions.
The supplemental patient data will also offer healthcare organizations a look into quality measures, performance, cost of care, risk distribution, and gap opportunities.
These insights aim to address social determinants of health, identify opportunities for improving outcomes, and better manage patient costs.

Population Insight includes risk algorithms like Innovaccer’s social vulnerability index,” said Kate Jenkins-Brown, MEDITECH product manager. “These insights provide greater agility to meet evolving patient needs.”
These advancements add to ongoing efforts by health IT organizations to improve population health management.
In early 2022 HEALTHeLINK, the health information exchange (HIE) for Western New York revealed , its participation in a total of 83 projects for use cases such as population health management.
In 2021, the HIE saw increased utilization of its community population health tool, HEALTHeOUTCOMES, which leverages HIE data and claims data from Highmark Blue Cross Blue Shield of Western New York, Independent Health, and Univera Healthcare.
