Dr. Nazanin Ghahreman-Falconer, the lead researcher, highlights that a staggering one-quarter of adult patients in Queensland are readmitted to hospitals within 28 days, imposing significant strain on the already overwhelmed healthcare system. Her research endeavors to employ predictive modeling to prevent rehospitalization following a heart attack. To achieve this, digital tools will be developed to identify cardiac patients with the highest risk of hospital readmission due to medication-related issues.
The project’s initial phase includes consultations with stakeholders and the creation of a medication management and support toolkit for discharged patients. Subsequently, the second phase will harness the power of artificial intelligence and Princess Alexandra Hospital’s data to pinpoint patients susceptible to medication harm.
