Immediate Test Results Spark Refresh Behavior
The 21st Century Cures Act policy granting patients immediate access to their test results has revealed an unexpected consequence: anxious portal refreshing. According to a new JAMA Network Open study, patients frequently refresh their patient portals while awaiting lab and diagnostic test results. Researchers suggest this behavior may indicate anxiety and highlights the need for healthcare organizations to address patient concerns proactively.
Digital waiting rooms have effectively replaced physical waiting areas for many patients. Instead of sitting nervously in a clinic, patients now experience that same anticipation while repeatedly checking their electronic portals for updates. This shift in patient behavior demonstrates how healthcare delivery changes can create new psychological experiences for patients.
Understanding the Cures Act Requirements
The information blocking rules under the 21st Century Cures Act took effect in April 2021, requiring healthcare organizations nationwide to make specific patient data immediately accessible. This includes clinician notes and test results.
The regulation was designed with patient rights in mind, operating on the principle that increased data access would enhance patient engagement and activation. However, many healthcare providers expressed concerns that unlimited access to this information might harm patient-provider relationships.
Provider concerns were specific: patients might experience heightened anxiety awaiting results and potentially misinterpret findings when viewing them without clinical guidance. These concerns reflect the delicate balance between transparency and the potential psychological impact of medical information when delivered without contextual support.
Prior to these regulations, healthcare organizations often implemented deliberate delays in releasing sensitive test results. This practice allowed clinicians to review findings first and prepare appropriate guidance for patients, especially when delivering challenging news. The shift to immediate release represents a fundamental change in information flow within healthcare systems.
Research Reveals Patient Behaviors
Researchers from Vanderbilt University Medical Center (VUMC) discovered compelling evidence of worry-indicating behaviors through portal usage analysis. Their examination of VUMC’s patient portal activity among adult patients between January 2022 and December 2023 revealed:
- 37.2% of all patients refreshed their portal while awaiting results
- 39.3% refreshed when awaiting highly sensitive test results
- Patients with enabled notifications were more likely to refresh for high-sensitivity results
- Frequent refreshing correlated with higher rates of provider messaging
“Refresh activity for low-sensitivity results was more commonly associated with messaging,” the researchers noted. “This finding suggests that frequent refreshing, a possible surrogate measure of patient worry about a result, may be a characteristic of patient attitudes and preferences rather than of the test result itself.”
The digital behavior patterns identified in this study provide new insights into patient psychology during the waiting period. The act of repeatedly checking for results demonstrates how technology interfaces can reveal emotional states that might otherwise remain hidden from healthcare providers.
Balancing Patient Access and Anxiety
The researchers acknowledged that portal refreshing might not exclusively indicate anxiety. Alternative explanations include patients being proactive about their care or engaged in self-management practices.
Nevertheless, the connection between portal refreshes and subsequent messaging has significant implications. Healthcare organizations must prioritize strategies to alleviate patient concerns, particularly during result-waiting periods.
Meanwhile, clinician challenges mount as electronic health record (EHR) workloads increase, including the growing burden of responding to patient portal messages. Organizations focused on provider satisfaction need solutions to manage these messaging demands effectively.
Implementation of supportive resources could help address patient anxiety without increasing provider workload. Educational materials explaining common test results, expected timelines for follow-up, and when to be concerned about specific findings could alleviate unnecessary worry while empowering patients with knowledge. These resources might include automated explanations of results, links to reliable health information, or even AI-assisted interpretation tools that highlight when values fall within normal ranges.
Policy Solutions and Technical Innovations
The ability to identify anxious patients opens opportunities for healthcare organizations, policymakers, and health IT developers to create better experiences for both patients and providers.
Some states have already proposed or enacted legislation exempting highly sensitive results (such as cancer diagnoses) from information blocking rules. These exceptions would allow delays until clinicians can properly guide patients through understanding their results.
However, the research suggests personal preference might be more important than test sensitivity in predicting anxiety. This indicates patient-selected options for result delivery timing could be more effective.
Encouragingly, some health IT vendors have begun developing functionality allowing patients to personalize their result delivery preferences, which researchers consider a positive development for addressing these concerns.
Future directions for healthcare technology could include more sophisticated notification systems that detect patterns of anxious behavior and trigger appropriate support mechanisms. For instance, systems might offer proactive messaging or resources to patients exhibiting signs of concern before they reach out to providers. Such innovations could simultaneously improve patient experience while protecting clinician time and attention for cases requiring direct intervention.
