Together, they aim to resolve the data portability and interoperability challenges that have long fragmented American healthcare.

Furthermore, CMS outlined a voluntary interoperability framework requiring participating entities to:

  • Provide FHIR API-based data access by July 4, 2026
  • Support chart notes and clinical documents as FHIR attachments
  • Enable appointment and encounter notifications using FHIR subscriptions

Entities meeting these criteria become “CMS Aligned Networks,” demonstrating that they can securely exchange health data at scale.

b.well and Humana: Real-Time Data for Members

Breaking Down Data Silos for Insurer Members

Humana — one of the largest health insurance providers in the United States — has gone live with b.well to fulfill its commitment to the CMS Health Technology Ecosystem. Through this integration, Humana members can now access their health data across providers, health plans, pharmacies, and digital health apps — all in one centralized platform.

Moreover, this collaboration gives Humana real-time access to member data at the point of claims processing. Consequently, the insurer can respond faster to data requests from providers and other health plans. This supports critical use cases such as care coordination and quality improvement.

Caraline Coats, Senior Vice President of Provider Strategy and Operations at Humana, highlighted the significance of the partnership: “We’re improving access to health information which, in turn, helps free up time and resources so providers can focus on delivering care.”

Historically, patients have struggled to gather records spread across multiple systems. Now, Humana members gain a unified view of their medical history. This simplifies decisions, improves care coordination, and reduces the administrative burden on both patients and providers.

Noom Health Record Connect: Personalized Weight Loss

A New Feature for Medicare Beneficiaries

Noom — the leading behavior change company — has partnered with b.well to launch Noom Health Record Connect. This new, optional feature allows Medicare beneficiaries to securely verify their identity and connect their existing clinical health records to the Noom app. Notably, the integration also uses CLEAR, the secure identity platform, for compliant identity verification.

Once connected, Noom uses patient data — including diagnoses, medications, and lab results — to automatically personalize each member’s experience. The app then surfaces the most relevant programs, content, and tools based on each person’s specific health profile.

Free Trial for Medicare Members

To celebrate the launch, Noom is offering its flagship Noom Weight program completely free for 30 days for all new Medicare members. This demonstrates Noom’s commitment to making personalized weight loss, prediabetes management, and diabetes support accessible to more people.

Geoff Cook, CEO of Noom, explained the core challenge these partnerships address: “One of the biggest barriers to truly personalized healthcare is data silos. A Medicare member can now connect their health records to Noom in minutes.”

How b.well Powers the Noom Experience

b.well’s national health data network provides the backbone for this integration. Consequently, Noom can access authorized patient records that span millions of providers and health plans. Together, b.well and CLEAR make data sharing both secure and seamless for Medicare users.

b.well and Welldoc: Smarter Cardiometabolic Care

Bringing Patient Data Into Chronic Condition Apps

Welldoc offers an AI-enabled application for cardiometabolic health, supporting people living with conditions such as diabetes and heart disease. Through its new partnership with b.well, Welldoc can now access patient-authorized health data directly through b.well’s national health data network.

As a result, Welldoc users gain access to their comprehensive health information within the app. This enables more accurate, personalized guidance for managing cardiometabolic conditions. Instead of relying on incomplete or outdated records, Welldoc can draw on normalized, real-time data to power its AI-driven recommendations.

The Role of Interoperability in Chronic Disease Management

Chronic conditions like diabetes require consistent monitoring and personalized intervention. However, fragmented data has long limited the effectiveness of digital health tools. This partnership directly addresses that gap. Therefore, patients with cardiometabolic conditions stand to benefit most from this improved data connectivity.

Interestingly, b.well’s CEO Valdes revealed that companies participating in the CMS Interoperability Framework communicate in real time over Slack. This direct communication channel accelerates problem-solving on technical and operational challenges. Participants use these channels to pitch, analyze, and co-develop tools for patient matching, data exchange standards, and other interoperability needs.

What’s Next for Health Data Interoperability

Building Toward a Connected Healthcare Future

The CMS Health Technology Ecosystem has already gathered more than 700 organizations, with over 120 ready or nearly ready for production. Acting Administrator Amy Gleason described the initiative’s ambition: the goal is “freedom from the clipboard, and finally, freedom for patients to access and use their own health information.”

Thomas Keane, National Coordinator for Health IT, added that the innovations developed through this initiative will be adopted into TEFCA and ONC’s standards and certification programs. Consequently, the progress made here will shape healthcare data exchange policy for years ahead.
